Care Navigation and Coordination Support

Navigating financial systems, insurance requirements, and public benefits can be one of the most stressful parts of caregiving. Antlers Creek Caregiver Foundation supports caregivers by helping them navigate and coordinate next steps—so they don’t have to face these systems alone.

Our role is supportive and coordination-based. We help caregivers understand who to contact, what steps may be needed, and how to move the process forward—while ensuring licensed professionals and appropriate agencies remain responsible for assessments, decisions, and approvals.

How We Support Caregivers

CONNECTING

Connecting caregivers with Medicaid consultants and Single Entry Point (SEP) agencies that complete required level-of-care assessments.

COORDINATING

Coordinating with care providers to help ensure required paperwork is completed and routed to the appropriate agencies

ASSISTING

Assisting caregivers in contacting long-term care insurance companies and helping ensure provider documentation reflects the need for ongoing care

All support is provided with appropriate permissions and in close coordination with caregivers, providers, and partner organizations.
